Vaccine Verification

To protect the health and safety of our community, we are asking all those who participate in Beth Tzedec programs and services to verify their COVID-19 vaccination status.

As of February 14, 2022:
  • No one 12 years or older will be permitted to enter the Beth Tzedec building or grounds without first verifying that they have been fully vaccinated against COVID-19 at least 14 days prior to entry with a vaccine approved by Health Canada.
  • Children ages 5 to 11 (or an adult on behalf of such a person), must verify that they are fully vaccinated with a vaccine for COVID-19 approved by Health Canada at least 14 days prior to entry.
  • Children under the age of 5 are not required to verify their vaccine status.

Once your vaccination receipt(s) have been reviewed, they will be deleted from our records. If you would like to review Beth Tzedec's Privacy Policy, it is available here>>
Please upload BOTH of your COVID-19 vaccination receipts. If you were vaccinated in Ontario, you can download your receipts here>>

To access your vaccination receipts, you should have your provincial health card available. Please note that you will not be able to download your vaccination receipts if you still have a red and white health card. 

Individuals with a red and while heath card can call the Provincial Vaccine Information Line number at 1-888-999-6488 to request a copy of their receipts. 
Please upload at least ONE of your child's COVID-19 vaccination receipts. If they were vaccinated in Ontario, you can download their receipts here>>

To access vaccination receipts, you should have your child's provincial health card available.
You can upload a maximum of 2 files.

We will be in touch if there are any issues with your upload(s). Please allow at least two business days for us to review your submission before attending any in-person programs or services at Beth Tzedec.
